Simply submit the following claims requirements:

  • Fully accomplished claimant’s statement form
  • Copy of Claimant’s Valid Identification Document (Photo-bearing and valid, government-issued with 3 specimen signatures)
  • Attending Physician’s Statement (APS) for the specific illness being claimed (Should be accomplished by a Medical Specialist along with confirmatory test results)
  • Complete Medical Records including but not limited to the following:
    • Admission History
    • Clinical Abstract
    • Laboratory Test Results
    • Record of Operation
